Male infertility/fertility

As a niche within the specialty of urology, male infertility is still in its relative infancy. Together with the field of female infertility (reproductive endocrinology) urologists have made great strides in treating couples who, even 10 years ago, would have been deemed infertile beyond hope of help. The fellowship training programs for male infertility specialists are few in number and most major metropolitan areas have one or two urologists that are specially trained to tackle the unique set of issues and problems associated with treating the infertile man and hence the infertile couple.

The study and treatment of male infertility is a discipline that cuts across all aspects of urology from cancer to infections to erectile dysfunction, from neurologic problems of the urinary tract to hormonal issues. Male infertility treatments are wide ranging from technically demanding microsurgery to hormone replacement and supplementation.

The importance of evaluating men cannot be overstated. It is often the female partner who initially presents to a medical professional for an infertility evaluation, and it is largely a cultural bias to expect that women bear the sole responsibility for this reasonably prevalent problem. We know, however, that the exact opposite is true. A male factor plays a significant role in a couple's infertility roughly half the time. Furthermore, a number of other health considerations, some of them life-threatening, can have infertility as their primary manifestation.

The cornerstone of a basic male infertility evaluation includes a thorough history and physical examination and a semen analysis. Laboratory and radiology tests are generally reserved for a more detailed level of evaluation if indicated.

Infertility is a complex and oftentimes frustrating problem which affects couples on many different levels. My philosophy, in a nutshell, is to help you achieve a pregnancy in as natural a way as possible, taking into account your emotional, ethical, and financial constraints.
