Office Policies

1. Patients must bring a referral at the time of visit if one is required by your insurance company. We are unable to obtain referrals while you are waiting in our office or retroactively.

2. Patients without a referral may reschedule or may be seen out of network and will be responsible for payment in full at the time of visit. You may be subject to a late cancellation fee if your visit is rescheduled.

3. Prescription refill requests or lab results are not provided by phone or fax. All such requests must be handled in person in the form of an office visit.

4. Co-payment is due at the time of visit without exception.

5. We accept Mastercard/Visa, cash and check.

6. Late cancellation policy –

   1. Office visits – $35 for no-shows or cancellations less than 24 hours.

   2. Procedures – $100 for cancellations within 72 hours.

7. Medical Records Requests: All requests require payment of $30 for patients or doctors, $100 for insurance companies or attorneys. Payment is accepted in the form of cash or check. Make all checks payable to Urologic Surgeons of Washington, 2021 K St NW, Ste 408, Washington, DC 20006. Requests must be accompanied by a signed Medical Records Release form, which are to be submitted along with payment. We will process the request within 48 hours of receipt of payment.

8. Please bring any pertinent x-rays or lab results with you to your visit. It is of utmost importance to bring the ACTUAL FILMS and not simply a written report so your doctor can go over them with you.

9. Patients with HMO plans have two options regarding labwork:

   1. You may go to your pre-approved laboratory and schedule a follow-up visit to go over results.

   2. You may sign an insurance waiver and pay to have labs preformed in our office.

10. Employee leave forms or letters: Please mail or fax your leave forms to your doctor's assistant at Urologic Surgeons of Washington, 2021 K St NW, Ste 408, Washington, DC 20006 with attention to Ruby or Beverly. Please include payment of $30 for one page, or $50 dollars for forms two pages or greater. Your request will be processed within 48 hours of receipt of payment. Payment is accepted in the form of cash or check.
